The ‘Set Up An Appointment’ button will link you to Dr. Rahman’s Practice Fusion profile page. Simply choose an appointment day and time, enter your information, and you’re done! You will receive an email confirming your appointment request. After booking an appointment, make sure to download both the patient and consent forms.
